Care management for families

Care management, without a care manager.

Care management is the work of keeping someone's care coordinated: the right appointments, the right medications, the right people informed. Hospitals have care managers for it. Families usually have one tired person. Titan.Care is built to help that person.

What care management means at home

In a hospital, a care manager makes sure nothing falls between specialists. At home the same gaps exist: a new prescription from one doctor that another does not know about, a follow-up nobody booked, a pre-op visit before a surgery, a result that needs a call.

  • Every appointment, from every hospital, on one list per person
  • Medication changes noticed and shown to the family
  • What still needs doing before a big day, with a name against each item

Coordination across the family

Care is rarely one person's job. Titan.Care's care circle lets siblings, partners and paid caregivers work from the same record, each with the access you choose, so the coordinating happens in one place instead of a group text.

Ready for the appointment

Before a visit, Titan.Care can write a Doctor Handoff: the recent history, current medications and open questions, on one page to bring with you. After it, the visit summary arrives from MyChart and the family sees what changed.

Questions families ask

Is Titan.Care a care management platform for agencies?

Titan.Care is built for families. It gives one family the coordination tools a care manager uses, around the records the hospitals already hold.

Can a paid caregiver use it?

Yes. Invite them into the care circle and choose what they can see, for example the calendar and medications but not the documents.

Does it replace my doctor's advice?

No. Titan.Care organises information and helps you prepare. It does not give medical advice, diagnosis or treatment.

How much does it cost?

Free for two people and one hospital. Titan.Care Plus is $24.99 a month for four people and every hospital.
