MyChart for families

Your mom's MyChart, your dad's MyChart and yours, in one place.

MyChart was built for one patient at one hospital. Families do not work that way. Here is how to keep track of everyone's MyChart without logging in and out of five portals, and how to bring the rest of the family along.

The problem with MyChart when you care for someone else

Each hospital runs its own MyChart. If your mother sees a cardiologist at one health system and her primary care doctor at another, she has two accounts, two passwords and two medication lists that do not quite agree. Add your own family and a child's pediatrician, and it is easy to end up with six portals.

MyChart's proxy access helps you see someone else's chart inside that one hospital's portal. It does not combine hospitals, and it does not help your brother, who also wants to know what the doctor said.

Connect once, and the records come to you

In Titan.Care you connect each MyChart once, signing in on the hospital's own page. We never see or store the password. From then on, appointments, medications, lab results and documents from that hospital arrive in that person's record on their own.

Titan.Care works with the MyChart of more than 800 health systems that use Epic, from large academic hospitals to family practices.

  • Sign in on the hospital's own MyChart page; your password stays with them
  • Appointments, medications, results and documents sync automatically
  • Disconnect any hospital at any time

Bring the family in without sharing passwords

Instead of passing a MyChart password around the family, invite relatives into the care circle. Each person sees what you allow, from the whole record to just the calendar, and you can change it at any time.

Questions families ask

How do I connect my parent's MyChart?

Sit down with your parent once, choose their hospital in Titan.Care, and sign in with their MyChart on the hospital's own page. Their records then sync to their profile in Titan.Care, and you can invite the rest of the family.

What is MyChart proxy access?

Proxy access is a hospital's way of letting you see another person's chart inside that hospital's MyChart, usually after a form signed by the patient. It is useful, but it only covers that one hospital.

Is Titan.Care made by Epic?

No. Titan.Care is an independent company. It connects to MyChart through the standard patient access that Epic provides to apps the patient chooses. MyChart and Epic are trademarks of Epic Systems Corporation.

Do you store my MyChart password?

Never. You sign in on the hospital's own page, and the hospital gives Titan.Care permission to read the records. You can withdraw it from Titan.Care or from MyChart.
