Family health records

Your family's health records, in your hands for once.

Hospitals keep your records in an EHR or EMR that you cannot see whole. A personal health record (PHR) is your own copy. Here is the difference, and how to build one for everyone in your family without typing anything in.

EHR, EMR and PHR in plain words

An EMR (electronic medical record) is the chart one practice keeps about you. An EHR (electronic health record) is the same idea across a whole health system, built to be shared between its doctors. Epic is the most widely used EHR in the United States, and MyChart is the window it gives patients.

A PHR (personal health record) belongs to you. It pulls from all of those systems and keeps one record for each person, which you control and can share with whoever you choose.

A record that builds itself

Old PHR apps asked you to type in your history. Titan.Care builds the record from the hospitals: connect MyChart and the documents arrive on their own. After-visit summaries, progress notes, imaging studies, lab reports, discharge instructions and more, in one list, newest first.

  • Documents from every connected hospital, in one list
  • Lab results with their reference ranges, flagged when out of range
  • A plain-language brief of what changed, for anyone in the family

One record for each person you look after

A family health record is more than your own chart. Titan.Care keeps a record for each person, a parent, a child, a partner, and lets you choose who in the family can see which one.

Questions families ask

What is the difference between an EMR and an EHR?

An EMR is the record one practice keeps. An EHR covers a whole health system and is designed to be shared between clinicians. Most hospitals today run an EHR, such as Epic.

What is a personal health record?

A record the patient or family controls, gathered from the hospitals' systems. Titan.Care is a personal health record for the whole family.

Do I have to enter my medical history?

No. Connect MyChart and the records arrive by themselves. You can add notes, documents and appointments of your own too.

Can I share the record with a doctor?

Yes. Titan.Care can prepare a Doctor Handoff, a summary to bring to an appointment, and you can share a Care Brief by link.
