Combine MyChart accounts
Many hospitals, many MyCharts. One record.
If you have ever kept a list of which MyChart belongs to which hospital, this page is for you. Here is why the accounts stay separate, what Epic's Happy Together does and does not do, and how to see everything in one place.
Why you have more than one MyChart
MyChart belongs to the hospital, not to you. Every health system that runs Epic hosts its own MyChart, so a patient who sees doctors at three systems has three accounts. That is normal, and it is the reason a medication list at one hospital often misses what another one prescribed.
Some hospitals offer Epic's Happy Together, which shows another organisation's information inside their MyChart. It depends on both hospitals taking part, and it still means one account per person.
One record per person, built from every MyChart
Titan.Care connects to each MyChart separately and builds a single record for each person from all of them: one list of appointments in date order, one medication list, every document newest first, and lab results flagged when they fall outside the range.
- Appointments from every hospital on one list, with where each came from
- Duplicates caught when the same visit arrives twice
- Lab results from every system, flagged when out of range

And then the whole family
Most people who have three MyCharts are also keeping track of somebody else's. Titan.Care holds a record for each person you look after, so your father's hospitals and your son's pediatrician sit side by side, and the family calendar shows all of it.

Questions families ask
Can I merge two MyChart accounts?
Not inside MyChart: each account belongs to a different hospital. Titan.Care connects to each one and shows them together as one record, without changing either account.
What is Epic Happy Together?
A feature some hospitals turn on to show another organisation's records inside their MyChart. It only works where both hospitals take part, and you still need an account at each.
How many hospitals can I connect?
On the free plan, one hospital. Titan.Care Plus connects every hospital for everyone you look after.
Will my records update on their own?
Yes. After you connect, new appointments, medications, results and documents arrive automatically.
