Medical records in plain words
The whole record, explained in plain words.
Hospital records are written for clinicians: codes, abbreviations and results with no story attached. Titan.Care reads what came in from every connected hospital and writes it up the way you would explain it to your sister on the phone, with the original record one tap away.
A Care Brief: what changed, in plain language
A Care Brief pulls the latest visits, results, medications and notes from every connected hospital into one page anyone in the family can read. It says what changed, what is new on the medication list and what is worth asking about next time.
Points link back to the document or lab result they came from, so nothing has to be taken on trust.

A Doctor Handoff: one page for the next appointment
A new specialist or an ER doctor at a different hospital often cannot see what the others did. A Doctor Handoff puts the recent history, current medications from every hospital and your open questions on one page you can show or share.

New documents, explained when they arrive
On Titan.Care Plus, visit notes, imaging reports and letters are explained in a few plain sentences as they come in, so a parent's new report does not sit unread until the next visit.
- Works across every hospital you connect through MyChart, not just one
- Seen only by the family members you choose
- Titan.Care asks before it reads anyone's records, and you can say no

What it does not do
A summary is a reading aid, not a diagnosis. Titan.Care does not give medical advice or tell you which treatment to choose. When something in the record worries you, the summary helps you ask the doctor a sharper question, and the doctor answers it.
Questions families ask
Can AI summarize my parent's medical records?
Yes. Titan.Care connects to the hospitals your parent uses through MyChart and writes a plain-language Care Brief from what comes in, with points linked to the original documents and results.
Is the summary a diagnosis?
No. Titan.Care explains what the record says and suggests questions to ask. It does not give medical advice, diagnosis or treatment. Always check with the doctor.
Who can see the summaries?
Only the people in that person's care circle. Records and summaries are never sold or used for advertising.
Does it work with more than one hospital?
Yes. Connect each hospital's MyChart once and Titan.Care summarises them together, which is where a summary helps most.
How many summaries are included?
Titan.Care Plus, $24.99 a month, includes six Care Briefs and six Doctor Handoffs a month. The free plan includes one of each to try.
